$20 Sports Physicals July 31!

Wednesday July 31st from 9:00am to 4:00pm Dr. Terry Ramos will be performing sports physicals on a walk-in basis for $20.  If you are having a school physical done as well, she requests that you come to the appointment with the sports physical form in hand and would like the parent section completed prior to arrival.  Make sure you bring our medical release form as we cannot accept school physical forms.
